“That’s a risky roll of the dice,” says one longtime cannabis researcher about taking CBD as protection from COVID-19.

Perhaps it wasn’t surprising that ads touting cannabidiol (CBD) would spring up during the COVID-19 pandemic. After all, the cannabis derivative has been touted in recent years for its antiviral properties.

But this doesn’t mean it’s effective against the novel coronavirus. And for people who take certain medications, it may not be safe.

A Look at the Evidence (0r Lack Thereof)

CBD might one day prove to be helpful for people who have or who are recovering from a viral illness, but the current data don’t support its use with the coronavirus, says Jahan Marcu, PhD, a longtime cannabis researcher in New York City and editor in chief of the American Journal of Endocannibinoid Medicine.

“If you’re using CBD as a shield to protect yourself against the coronavirus, that’s a risky roll of the dice right now,” he says.

Those who study CBD have been alarmed by some of the claims being made by advertisers. The board of the International Association on Cannabinoid Medicines (IACM) put out a statement in March asking members not to amplify “false information that is circulating on the Internet.”ADVERTISINGSunil K. Aggarwal, MD, PhD, an integrative medicine physician who uses cannabis in his Seattle practice, notes that there is no evidence that CBD protects against coronavirus infection or effectively treats it, the same message the IACM is working to convey.

What Exactly Is CBD?

Cannabis plants, which include both marijuana and hemp varieties, contain hundreds of different compounds. The most famous is tetrahydrocannabinol, commonly known as THC, which is the chemical that makes you high. Another is CBD, which does not create a high.

Hemp plants contain negligible amounts of THC, a key reason why companies have been able to sell a broad array of hemp products. CBD has been especially popular in recent years, with an explosion of offerings featuring the substance in everything from pills and tinctures to coffees and skin cream.

Many CBD products are sold online, with a growing number also available from dispensaries in states that have legalized medical or recreational cannabis.

There’s Very Little Human Research

Despite CBD’s widespread availability, hardly any research has been conducted in human subjects. This is because laws governing the legality of cannabis make these investigations difficult.

The most widely researched use for CBD is for epilepsy, with the U.S. Food and Drug Administration (FDA) approving the first CBD-based drugEpidiolex, in 2018 to treat rare forms of the disease. The drug’s manufacturer, GW Research, tested it in hundreds of patients before getting approval.

For most other uses, much of the research has involved laboratory animals and test tubes. It is here that CBD has shown promising antiviral properties, as well as antibacterial and other effects, Dr. Marcu says.

CBD may also relieve anxiety and help you sleep, according to a tiny study of 15 people published in the Journal of Clinical Pharmacology back in 1981. Researchers found that patients with insomnia taking 160 milligrams of CBD before bed (but not lower doses) slept longer than those taking a placebo.

But as the IACM notes, no human studies have been published regarding CBD’s effects on the coronavirus.

CBD Is Not Side-Effect Free

Marketers touting CBD often point to a 2017 statement by the World Health Organization (WHO) that says in its pure state, “cannabidiol does not appear to have abuse potential or cause harm.”

But they rarely mention the rest of the WHO statement — that the organization “does not recommend cannabidiol for medical use.”

The chemical is not side-effect free, with people taking CBD reporting problems including diarrhea, fatigue, and vomiting, according to a 2019 review in Current Neuropharmacology.

Potential Drug Interactions

For people with chronic health conditions, another issue is the possibility that CBD might interact negatively with certain medications.

A report published on the U.S. National Library of Medicine site warns people on the seizure medications clobazam (Onfi and Sympazan) and valproic acid (Depakene) not to take CBD, since it can increase these drugs’ side effects or boost the risk of liver injury.

For many other medicines, CBD can impact how much of the drug circulates in the body. “CBD targets the same metabolizing enzymes in the liver as many common medications,” Marcu says. “If you’re on other medicines, you must proceed with caution.”Speak with your physician before taking CBD if you’re taking a drug such as a benzodiazepinesedative, the pain medicine celecoxib (Celebrex), the gastrointestinal (GI) drug omeprazole (Prilosec, Omesec), the heart pill nifedipine (Procardia), or many others.

If You Use CBD, Buy Only Quality Products

If you have consulted with your physician and decide you still want to use CBD, it’s crucial that you buy it from a reputable manufacturer, experts say. You’ll want a company that tests each batch of product and lists the batch number on the bottle itself, so you can link it to corresponding test results on the company’s website or via a phone call to customer service.

One giveaway a product isn’t up to snuff: It’s too cheap. Good CBD is costly, with a bottle of tincture typically selling for $60 or more. 

As for how much to take, your guess is as good as any. There haven’t been studies on the various dosages, so nobody really knows, Dr. Aggarwal says.

COVID-19 Lung Inflammation Reduced by CBD

The Dental College of Georgia (DCG) and the Medical College of Georgia, recently performed a study which produced results showing that cannabidiol (CBD), a non-psychoactive cannabinoid found in cannabis, reduces damage in the lungs spurred by cytokine storms caused by coronavirus (COVID-19).

A recent study from the Dental College of Georgia (DCG) (Augusta, Georgia) and the Medical College of Georgia (Augusta, Georgia) produced results showing that cannabidiol (CBD), a non-psychoactive cannabinoid found in cannabis, reduces damage in the lungs spurred by cytokine storms caused by coronavirus (COVID-19) (1). In an earlier study (2), the researchers illustrated that CBD can reduce inflammation and physical lung damage associated with adult respiratory distress syndrome (ARDS), along with improved oxygen levels. The new study revealed the mechanisms behind these earlier results, exhibiting that CBD normalizes levels of apelin, a peptide known to reduce inflammation. During a COVID-19 infection, levels of the apelin peptide are low. Results of this study have been published in the Journal of Cellular and Molecular Medicine (3).

In the authors ARDS model, blood levels of apelin (a crucial piece in regulation, bringing both blood pressure and inflammation down) had dropped close to zero and increased 20 times with CBD. In the instance that blood pressure gets too high, apelin levels rise up to help alleviate the pressure. It is assumed that apelin should also help normalize inflammation in the lungs and other related breathing difficulties linked to ARDS. “Ideally with ARDS it would increase in areas of the lungs where it’s needed to improve blood and oxygen flow to compensate and to protect,” said Dr. Babak Baban, DCG immunologist (1). In their ARDS model, apelin did neither of these. When CBD was administered, apelin increased.

Although with their current study, the authors reiterated that much more is still being studied and there is some animosity in regard to whether CBD or COVID-19 have a direct effect on apelin but they are continuing on their mission to find out. “It is an association; we don’t know yet about causative, but it is a very good indicator of the disease,” Baban said (1). What they have found so far is that the reductions in apelin could potentially be an early biomarker for ARDS and response to treatment efforts.

Based on their earlier study, the authors mentioned that the data supported the idea that the anti-inflammatory function of CBD may assist in reducing cytokine storm and mitigate the effects of exaggerated inflammation (2). Further adding that, “considering all potential regulatory effects of CBD as well as the vast distribution of endocannabinoid system in the body, it is plausible that CBD may be used as a therapeutic candidate in the treatment of various inflammatory conditions including COVID-19 and other virus-induced ARDS.”

Expanding from the researchers’ newest study, the next steps for their research would be to comprise a stronger understanding of the interactions between apelin, CBD, and COVID-19 as well as studying why apelin decreases during COVID-19 infections and how CBD increases those numbers. They will explore how eliminating apelin affects ARDS and if CBD is able to produce the same lung benefit without the peptide. It is assumed that COVID-19 suppresses some sort of reaction that then suppresses apelin and CBD interferes with this process. Nonetheless, the authors doubt the apelin-CBD interaction is the only means by which the compound works in this or other scenarios.

Moving forward with their studies, the authors have refined a safe, relatively inexpensive model of ARDS by giving a synthetic analogue of double-stranded ribonucleic acid (RNA) called POLY (I:C). COVID-19 has double-stranded RNA, while human deoxyribonucleic acid (DNA) is single-stranded. The researchers’ model propagated a response similar to the virus, along with the extreme lung damage and cytokine storm, reflecting an overactive immune response in the lungs.

Gathering data for these studies, a control group received intranasal saline consecutively for three days while the COVID-19 model received POLY (I:C) intranasally for three days. Then there was a third group, known as the “treatment group,” who received POLY (I:C) and CBD over the same timeframe. The results produced significantly reduced apelin levels in the mice who had developed COVID-19-like symptoms compared to controls, and treatment with CBD normalized the immune response, apelin levels, oxygen levels, swelling, and scarring in the lungs.

CBD has shown itself to be a natural apelin agonist. Apelin levels are consistently measurable in the lungs, which is one of the reasons it proves to be a good biomarker. Aside from CBD, other cannabinoids, such as tetrahydrocannabinol (THC), are proving themselves beneficial in the fight against COVID-19 by decreasing the risk of cytokine storms, ARDS, and mortality (4,5). With future research, cannabis may prove itself useful in strengthening the side of those battling the virus and others fighting to rid the world of COVID-19.

Interestingly, there have been several different ways researchers have suggested CBD might help, including its ability to reduce ACE2 expression and pro-inflammatory cytokine production to fight lung inflammation, and it’s potential as an antiviral. But few studies have tested these theories with actual experimentation.  

Now new evidence is adding support to the theory that cannabis derived CBD may help those suffering from the severe lung inflammation that occurs in more serious cases of COVID-19. The study from researchers at Augusta University in Georgia suggests  that CBD may positively impact ARDS or acute respiratory distress syndrome – a dangerous symptom in COVID-19 caused by an overactive inflammatory response. This is sometimes referred to as a ‘cytokine storm’. This created a cytokine storm, reduced the blood oxygen saturation by 10%, and even produced structural damages to the lungs in mice subjects. Then after the mice were experiencing the cytokine storm and coinciding respiratory distress, they were given CBD. And the results showed just how powerful CBD can be when treating ARDS. “These symptoms were totally or partially reversed and returned to the level and condition of the normal after treatment with CBD” the authors report.

In particular, mice treated with CBD saw reduced expression of IL-6, an important marker for cytokine storms, and lowered the levels of other proinflammatory cytokines. While the Poly:(I:C) spiked inflammatory markers, “CBD treatment reversed all these inflammatory indices and partially re-established homeostasis” the authors explain. Mice treated with CBD also had increases in lymphocyte levels in their blood, which are important white blood cells for fighting off infections. 

Researchers on the study say that CBD could play an immunotherapeutic role in treating severe respiratory viral infections like COVID-19, based on the present findings. 

“The current data support the notion that the anti-inflammatory function of CBD may reduce cytokine storm and mitigate the effects of exaggerated inflammation” they explain. They add that “considering all potential regulatory effects of CBD as well as the vast distribution of endocannabinoid system in the body, it is plausible that CBD may be used as a therapeutic candidate in the treatment of various inflammatory conditions including COVID-19 and other virus-induced ARDS.”

Of course, this is an early study, and more research needs to be done – particularly more research that tests this theory on actual human subjects with COVID-19 related ARDS. The authors point out that “obviously, more studies are required to expand and validate this therapeutic strategy.” 

While we may have to wait for more research before this is being used as an active treatment for COVID-19, this study gives us good reason to hope that CBD might eventually be harnessed to help fight ARDS. It also adds to the chorus of researchers and cannabis advocates suggesting that we look further into cannabis and its potential to fight this deadly disease.

The research itself was performed on mice, who were first given a treatment called Poly(I:C) to artificially induce ARDS, with symptoms similar to what we see in severe COVID-19 patients. This created a cytokine storm, reduced the blood oxygen saturation by 10%, and even produced structural damages to the lungs in mice subjects. In particular, mice treated with CBD saw reduced expression of IL-6, an important marker for cytokine storms, and lowered the levels of other proinflammatory cytokines. While the Poly:(I:C) spiked inflammatory markers, “CBD treatment reversed all these inflammatory indices and partially re-established homeostasis” the authors explain. Mice treated with CBD also had increases in lymphocyte levels in their blood, which are important white blood cells for fighting off infections. 

Researchers on the study say that CBD could play an immunotherapeutic role in treating severe respiratory viral infections like COVID-19, based on the present findings. 

“The current data support the notion that the anti-inflammatory function of CBD may reduce cytokine storm and mitigate the effects of exaggerated inflammation” they explain. They add that “considering all potential regulatory effects of CBD as well as the vast distribution of endocannabinoid system in the body, it is plausible that CBD may be used as a therapeutic candidate in the treatment of various inflammatory conditions including COVID-19 and other virus-induced ARDS.”

Of course, this is an early study, and more research needs to be done – particularly more research that tests this theory on actual human subjects with COVID-19 related ARDS. The authors point out that “obviously, more studies are required to expand and validate this therapeutic strategy.” 

While we may have to wait for more research before this is being used as an active treatment for COVID-19, this study gives us good reason to hope that CBD might eventually be harnessed to help fight ARDS. It also adds to the chorus of researchers and cannabis advocates suggesting that we look further into cannabis and its potential to fight this deadly disease.

In a new study, scientists have shown that one of the non-psychoactive cannabinoids in cannabis – CBD – reduces lung damage induced by cytokine storms caused by COVID-19 through enabling an increase in levels of a natural peptide.

The scientists, from the Dental College of Georgia (DCG) and the Medical College of Georgia, demonstrated earlier this year that CBD has the ability to improve oxygen levels and reduce inflammation and physical lung damage related to adult respiratory distress syndrome (ARDS). This new study has now shown the mechanisms behind these results, demonstrating that CBD normalises levels of a peptide named apelin, which is known to reduce inflammation. Levels of this peptide are low during a COVID-19 infection.

While the researchers say the peptide clearly has an important role to play, they do not attribute all CBD’s benefits to apelin. The results have been published in the Journal of Cellular and Molecular Medicine.

Reducing inflammation with CBD

Blood levels of apelin, which is an important regulator in bringing both blood pressure and inflammation down, dropped close to zero in the authors’ ARDS model and increased 20 times with CBD. When blood pressure gets high, for example, apelin levels should go up to help reduce the pressure. Apelin should do the same to help normalise inflammation in the lungs and related breathing difficulties associated with ARDS.

“Ideally with ARDS it would increase in areas of the lungs where it’s needed to improve blood and oxygen flow to compensate and to protect,” said Dr Babak Baban, DCG immunologist. But when the authors looked at their ARDS model, apelin did not do either, and instead decreased in both the lung tissue itself and the general circulation. However, this changed when they administered CBD.

The authors emphasise that they do not yet know whether the novel coronavirus or CBD have a direct effect on apelin, or if these are downstream consequences, however, they are already working to find out.

“It is an association; we don’t know yet about causative, but it is a very good indicator of the disease,” Baban said.

The authors note that the finding of dramatic reductions in apelin in the face of ARDS, makes levels of the protective peptide a potential early biomarker for ARDS and response to treatment efforts.

ACE2 receptor

The COVID-19 virus enters human cells via the also pervasive angiotensin-converting enzyme 2, otherwise known as the ACE2 receptor, with plenty of common ground between ACE2 and apelin, including the fact that many cell types and tissues have both, including the lungs.

Apelin and ACE2 normally work together to control blood pressure, and upregulation of both may be helpful in cardiovascular disease, including heart failure, by decreasing blood pressure while increasing the heart’s ability to pump.

The COVID-19 virus appears to upset this positive partnership, with the virus having the ability to bind to the receptor for ACE2. This has been shown to decrease ACE2 levels and increase levels of the powerful blood vessel constrictor angiotensin II, because less angiotensin II gets degraded and fewer vasodilators (chemicals that widen blood vessels) get produced, which worsens the patient’s prognosis.

“Instead of ACE2 helping blood vessels relax, it helps the virus get into the host where it makes more virus instead of helping the lungs relax and do their job,” says Yu.

CBD as a natural apelin agonist

The next steps in the research include better understanding the interaction between CBD, apelin, and the novel coronavirus, including why apelin goes down in the face of the virus and why CBD brings it up.

The researchers will be exploring how eliminating apelin affects ARDS and if CBD produces the same lung benefit without apelin. The authors note that it is likely the virus suppresses something that suppresses apelin and CBD interferes with this process, however, they also they doubt the apelin-CBD interaction is the only way the compound works in this and other scenarios.

To carry out the studies, the authors developed a safe, relatively inexpensive model of ARDS by giving a synthetic analogue of double-stranded RNA called POLY (I:C). The novel coronavirus also has double-stranded RNA, while human DNA is single-stranded. The model produced a response similar to the virus, including the extreme lung damage and the “cytokine storm” that reflects an over-the-top immune response in the lungs.

For these studies, a control group received intranasal saline for three consecutive days while the COVID-19 model received POLY (I:C) intranasally for three days. A third group, the treatment group, received POLY (I:C) and CBD over the same timeframe. The researchers found significantly reduced apelin levels in the mice that developed COVID-like symptoms compared to controls, and treatment with CBD normalised the immune response and apelin levels, along with oxygen levels, swelling, and scarring in the lungs.

“The apelinergic system is a very, very ubiquitous signalling system,” Yu says. The authors note that apelin has diverse jobs in different places and that its levels are consistently measurable in the lungs, one of the reasons it should be a good biomarker.

They add that CBD appears to be a natural apelin agonist.
